SVEIKATOS IR GEROVĖS PASLAUGOS
StreetSeen is the collective name for Light for Life’s interventions for individuals involved in street activity, whether that be Rough Sleeping, begging or street drinking/using substances. Light for Life provides Health & Wellbeing Services across the Sefton area for people with people with multiple and /or complex needs.
StreetSeen@St.Marksis a joint initiative between Light for Life and St. Marks Medical Centre, Southport. Working in partnership with St. Marks Medical Centre, we provide a specialist Health & Wellbeing Clinic for vulnerable people. This is a special GP led clinic providing access to a full range of clinical services for people who may find it difficult to engage with services due to their complex needs and lifestyles.
The service also provides in-reach support at Southport and Ormskirk NHS Hospital Trust for those at risk of homelessness to ensure appropriate support is in place on discharge to minimise repeat presentations.
Health & Wellbeing staff work alongside colleagues in both Health and Accommodation Services to bring about positive outcomes for those who are most vulnerable.
Health & Wellbeing Services include:
Access to GP, Mental Health Nurse and General Nursing
Accompanied appointments
On-going support for referrals to specialists/secondary health care services
Hospital In-reach Service
Support to engage with addiction and mental health services
Referrals for patients in A and E or admitted to wards who are homeless
Face to face assessments with patients for accommodation
Links into Social Care, Housing, Probation to ensure that patients have safe places to - go to on discharge
Ongoing support to reduce admissions and improve quality of life
